start to finish? depends on the job (how much revision/new artwork/rewrites, etc...) and whether or not the book is first edition. Usually about 15-18 months start to finish. Doctors are slow to submit chapters. And their handwriting, believe it or not, really does suck.

hey, i'm working on a plastic surgery book, too. good times. though, the images in mine are drawn, as opposed to pictures, so it's less gross.
