That Blur put not only the lyrics, but also the GUITAR CHORDS in the liner notes of Modern Life Is Rubbish.

That's cool.

Has any other band/artist done that?

Dunno, but The Fleshtones included the recipe for The Blue Whale as just about the only liner notes for The Living Legends comp, and The Chills included a recipe for some concoction called a "Sunburnt" in the album of the same name. After years of trying, I've given up trying to mix that thing.

Mogwai included the Cubase programme and the individual tracks for 'Hunted By A Freak' on their last album proper, so you could make your own remix.

That's pretty cool too.

When I first bought that Blur album and saw that, I thought I had seen it somewhere else but I cannot recall and after checking all of my cd booklets, figured it must have been a cd I no longer own.

Most of the stuff to which I listen can be cranked out with G, A, and D. Really don't need tablature for that :)
